Established clinical use Clomiphene citrate contains two geometric isomers. Enclomiphene, the trans isomer, is the antioestrogen at the hypothalamus: blocking oestrogen receptors there removes the brake on GnRH, so LH and FSH rise and the testis produces more testosterone. Zuclomiphene, the cis isomer, has oestrogenic activity and a half-life measured in days to weeks rather than hours, so it accumulates over sustained daily use.
That accumulation is the most likely explanation for effects that appear after several weeks on clomiphene rather than at the start — mood changes among them — and it is the entire rationale for the purified product. The app models enclomiphene at about ten hours against clomiphene’s blended five-day figure, and that difference is the zuclomiphene being gone rather than a change in what the active isomer does.
The mechanism is worth stating clearly because it is structurally different from testosterone therapy. LH and FSH rise rather than fall, testicular volume is preserved, and spermatogenesis is preserved or improved — which is why it is discussed as an option for men who want testosterone restored without losing fertility. Off-label or community practice Enclomiphene is not approved. A new drug application was filed and did not result in an approval, and what is available in the United States comes from compounding pharmacies. That places it in an awkward category: it is not a research chemical in the sense that most of this site’s Tier B compounds are — it has been through clinical trials in the intended population and the trial results are published — but it also has no approved product behind it, and no regulator has signed off on any particular preparation.
The practical consequence is the usual one. Identity, purity and content rest with the compounder, and there is no reference product to compare against. This site names no pharmacy and no testing service.
Off-label or community practice The trials reported testosterone rising into the normal range with sperm concentration preserved, which is the combination testosterone therapy cannot offer. What they did not show is testosterone reaching the levels people commonly target on testosterone therapy: this raises your own production within what your own axis can do, and for some men that ceiling is lower than they were hoping for. Setting that expectation before starting avoids a lot of disappointment.
Estradiol is the number to watch alongside it. Raising testosterone raises the substrate for aromatisation, and a SERM blocking receptors in the hypothalamus does not stop circulating estradiol rising elsewhere. The measured value and the receptor-level effect can move in different directions, which is exactly why the number alone does not settle anything — and why the assay has to be a sensitive one.
