Cabergoline: a long-acting dopamine agonist, and when prolactin actually needs treating

Highly effective at the thing it does, and prescribed for a number that is wrong more often than most. The pharmacology is simple; the difficulty is deciding whether the result that prompted it was real.

What it does, and why twice weekly

Established clinical use Prolactin release from the pituitary is under continuous inhibitory control by dopamine — unusually, the default state is suppression rather than stimulation. Cabergoline is a dopamine D2 receptor agonist that supplies that inhibition pharmacologically, and it is the first-line treatment for hyperprolactinaemia and for prolactin-secreting pituitary adenomas, where it both normalises the hormone and shrinks the tumour.

The app models its half-life at about two and a half days, which is very long for this class and is why a twice-weekly schedule works where other dopamine agonists need daily dosing. It also means the level accumulates for a couple of weeks before it settles, so an effect assessed at one week is being assessed before the drug has finished arriving.

Most high prolactin results are not what they look like

Established clinical use Prolactin is one of the easiest hormones to elevate by accident. It rises with the stress of venipuncture itself, with sleep, with exercise, with a meal, with nipple stimulation, and with a long list of common medications — antipsychotics and some antidepressants and antiemetics among them. A mildly high value on a single draw is more often one of those than a pituitary problem, which is why the standard response is to repeat it under controlled conditions rather than to treat it.

The other trap is macroprolactin: prolactin bound into large immune complexes that most immunoassays detect but which is biologically inactive. Someone with macroprolactinaemia has a genuinely high assay result and no disease, and the laboratory test that separates them has to be requested. Treating that patient achieves nothing except side effects. The prolactin page covers both problems and how the units differ between labs.

Off-label or community practice In hormone-therapy contexts, cabergoline is often used against prolactin elevated by progestogenic anabolic compounds — which this site does not publish pages for — or by higher-dose gonadotropin use. The app’s own protocol note is explicit that it should not be used prophylactically without a confirmed lab result, and that is the right instinct: this is a drug that treats a number, so the number has to be real first.

The valve question, stated in proportion

Established clinical use Ergot-derived dopamine agonists are associated with cardiac valve fibrosis, and the finding is real. What matters is the dose. The association was established in Parkinson’s disease, where cumulative doses run an order of magnitude or more above what is used for hyperprolactinaemia, and studies at endocrine doses have largely not found clinically significant valvulopathy. That is reassuring rather than conclusive, and it is why guidance for long-term use at higher endocrine doses includes periodic echocardiography.

The commoner effects are nausea, which is worst at initiation and usually settles, orthostatic dizziness, and fatigue. Impulse-control problems — a recognised class effect of dopamine agonists — are rare at these doses but worth naming, because the person experiencing one is often the last to attribute it to a tablet taken twice a week.

Questions people actually ask

How high does prolactin have to be before it means something?

That is a clinician call against your own lab’s interval, and it depends on how the sample was taken. What is standard is that a mildly high value is repeated under controlled conditions, and that macroprolactin is excluded, before anything is treated.

How long until it works?

Prolactin usually falls within days and normalises over weeks. Because the half-life is long, the level of the drug itself is still accumulating for the first couple of weeks, so early assessments underestimate the eventual effect.

Is an echocardiogram necessary?

Guidance generally reserves it for higher doses or long-term use, on the basis that the valve association was established at Parkinson’s-disease doses far above endocrine ones. Whether it applies to a particular person and schedule is a prescribing decision.

Does it affect testosterone?

Indirectly. High prolactin suppresses gonadotropin release and therefore testosterone, so correcting a genuinely elevated prolactin can raise it. Correcting a normal prolactin does nothing useful. The LH and FSH page covers that axis.
